| The original endorser can change the date later on the AffiliateEndorsement_nnnn_ form . For requested changes by others, a NYSBC scientific committee member may email dcadmin@nysbc.org the request. If this field is not set, the date will be automatically set up as 13 months from request. This will also be a default extension if none is specified in an extension request. | ||||||||
